Request a Tutor

This form must be completed if a subject area is not currently available in the Tutoring Center. If you are signing up for tutoring in more than one subject, you will need to submit a new form for each course.

1. Completing this form accurately is important. Your tutor (or the tutor coordinator) may need to call or e-mail you to let you know they are not available during your scheduled time.
2. You can sign up for 1/2 or 1 hour sessions with a maximum of 3 hours per week.
3. "No Shows" are taken seriously. If you cannot make an appointment, please call the Tutoring Center (343-0055 ext. 6204) as soon as possible. If you do not call 24 hours before your appointment, it is considered a "No Show"

Please keep in mind that when you sign up for a tutoring appointment, another student is prevented from signing up during that time. Additionally, if you do not give at least 24 hours advance cancellation notice, your tutor may be inconvenienced.
